Compensation Program Implementation Guidelines for YouBike Data Breach Incident

In response to the recent hacking incident where YouBike Company had its riding records stolen and subsequently used for fraudulent activities, YouBike, based on corporate social responsibility and moral obligation, intends to provide compensation to members whose riding records were compromised and who consequently suffered from fraud. The following guidelines outline the specific tasks and definitions related to the implementation of the compensation program.
A. General Case Compensation
1. Compensation Content:Equivalent to a NT$500 YouBike 2.0 Voucher.
2. Eligibility for Compensation:All members whose ride records were stolen in this incident.
3. Delivery Method:Directly deposited into the YouBike 2.0 App.
4. Distribution Time:The system development and distribution are planned to be completed by the end of September 2023.
B. Case Compensation
1. Compensation Content:50% of the defrauded amount, with a maximum limit of NT$20,000.
2. Process Stages:Divided into application stage, review stage, and distribution stage.
3. Application Criteria:The application for compensation must meet all of the following requirements:
a. Members who have been victimized by individuals impersonating YouBike and engaging in fraudulent activities.
b. Members who have completed the police reporting procedure regarding the fraud incident on or before May 28, 2023, 12:00 AM.
c. To support your claim, you will need to provide the following documentary evidence:
(1) A proof of handling issued by the police authority.
(2) Call records or text messages that demonstrate the time of receiving the fraud notification.
(3) The content of the fraud notification.
(4) Remittance receipts or other payment documentation.
(5) Any other necessary supporting documents.
(6) A photocopy of your identification card.
d. The supporting documents should be presented as photocopies or duplicates, which you can submit to YouBike company for the purpose of review.
e. The applicant should be the individual affected, and they are required to sign a personal data collection consent form.
f. If the applicant discovers any missing or incorrect information in their application, they should submit the necessary additional documents within three days from the date of submission. Failure to do so within the specified timeframe may result in the application being rejected.

5. Application process:
a. The application period is from June 1, 2023, to June 7, 2023
b. The service centers will accept applications during their designated working hours.
c. Upon submission at the service center, each application will be assigned a unique reference number. Members should use this reference number for any additional document submission or collection of compensation.
6. Review process:
a. If there are any missing or incorrect documents or information in the application, members will not be notified separately for rectification.
b. Once the review process is completed, the review results will not be amended.
7. Review Results:The review results will be notified via SMS within seven days prior to the disbursement start date.
8. Disbursement/Collection Period:From June 21, 2023, to June 30, 2023
9. Disbursement Method:
a. Payment Method:Immediate check payment.
b. Disbursement Counter:YouBike service centers.
c. Required Documents:Personal identification card/Health insurance card/Driver's license (two forms of identification).
d. The compensation will be received by the applicant in person, and a receipt will be issued upon collection.
10. YouBike Company reserves the right to interpret all the provisions within this document.
